Patient Forms
Once you’ve scheduled your initial evaluation, you will receive intake forms you can fill out on your computer or phone and email them to wellsmart.mh@gmail.com. You can also print and complete these forms and bring them with you to your first appointment.
Insurance
We accept Medicare, Medicaid (Medical Card), workers’ compensation, and most private insurance plans. Please call us at 304-476-9355 for more detail regarding insurance and fees. Our staff will verify and explain your benefits before treatment.
Pay Now
Make a payment on your account. Please click the “Buy Now” button to proceed with your payment. Once prompt, type in the amount due on your invoice. All payments are done securely via PayPal®.
