Patient Forms
Once you’ve scheduled your initial evaluation, you will receive intake forms you can fill out on your computer or phone and email them to wellsmart.mh@gmail.com. You can also print and complete these forms and bring them with you to your first appointment.
Insurance
We accept Medicare, Medicaid (Medical Card), workers’ compensation, and most private insurance plans. Take a look at the insurance providers we accept or make a payment.
